The Community Support Mentor plays a crucial role in delivering direct assistance to participants, facilitating their integration into the community and fostering independent living skills. A high school diploma or equivalent is required. We encourage individuals with disabilities to apply.

KEY RESPONSIBILITIES:

  • Ensure safe transportation for participants, including those utilizing mobility aids, to and from their residences and during community activities.
  • Implement the Participant's Staff Action Plan, developed collaboratively with the individual, their family, and the service planning team, focusing on their personal goals.
  • Support participants in achieving their goals while engaging in various community activities.
  • Address any issues that arise during daily activities with diplomacy and sensitivity.
  • Facilitate opportunities for participants to engage in chosen activities, ensuring adaptations are made to meet individual needs.
  • Assist participants in reaching their goals as outlined in their community habilitation plans.
  • Provide valuable feedback to the Employment & Family Services Director and Program Coordinator regarding participant goals.
  • Work alongside participants in community settings to promote skill development and independence.
  • Demonstrate empathy towards the personal needs of individuals and their families, responding appropriately.
  • Utilize the AWARDS database for timely documentation of services and maintain accurate daily and monthly records as per program regulations.
  • Complete all mandatory OPWDD and Independent Living training, including annual refreshers.
  • Engage in regular meetings with the Community Habilitation Coordinator and team members to ensure services align with participant needs and preferences.
  • Note that this list of responsibilities is not exhaustive; additional duties and training may be assigned as necessary.

QUALIFICATIONS:

  • High school diploma or equivalent.
  • Preferred experience working with individuals with developmental disabilities.
  • Spanish language skills are a plus.
  • Valid NYS Driver License with a clean driving record and proof of current auto insurance.
  • Strong written and verbal communication skills.
  • Familiarity with community resources.
  • First Aid and CPR certification.
  • Dependability, flexibility, a positive attitude, and strong organizational skills are essential.
  • Commitment to honesty, respect for diversity, and adherence to high personal standards reflecting the values of Independent Living.

WORK SCHEDULE:

  • Varies based on the needs of the participants.

COMPENSATION:

  • $16.00/hr - $21.08/hr
