As a Behavioral Health Coordinator at SSM Health, you will play a critical role in coordinating the department's involvement in complex care and case management of individuals with significant behavioral health concerns.
Key Responsibilities- Coordinate services for individuals with behavioral health needs who require complex case management support.
- Support standard patient access initiatives across the healthcare continuum.
- Coordinate behavioral health treatment and communications with behavioral health providers, patients, and families.
- Document and track department progress compared to goals and monitor success of department interventions.
- Maintain relationships with ministry contacts, community resources, referral sources, agencies, and associations pertaining to department.
- Facilitate meetings and represent department on community collaborations.
- May coordinate education and development of internal staff to ensure understanding of intake review and approval process.
- Ensure department operates within scope of practice.
- Work in a constant state of alertness and safe manner.
- Master's Degree in Behavioral Health Field.
- Three years of experience.
