Job Summary:The LPN- Visiting Hospice Nurse will be responsible for providing skilled nursing care to patients in their homes, focusing on pain management, symptom control, and emotional support. This is a per diem position, and the ideal candidate will have a strong nursing background, excellent communication skills, and a passion for delivering compassionate care.
Responsibilities:- Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
- Assess patients' needs and develop individualized care plans to meet their unique requirements.
- Collaborate with the interdisciplinary team to ensure comprehensive care and support for patients and their families.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Document patient care and progress in a timely and accurate manner.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Graduation from an accredited school of Practical Nursing and licensure to practice as a Practical Nurse in the State of New Jersey.
- CPR/AED Certification.
- Minimum one-year adult or pediatric medical/surgical clinical experience.
- Prior home care or hospice experience preferred.
