Key Responsibilities:
  • Provide medical care and management of terminal illnesses, ensuring compliance with state Nurse Practice Act and collaborating with physicians.
  • Evaluate patient health history, perform physical assessments, and develop individualized plans of care.
  • Prescribe medications, coordinate care services, and collaborate with the interdisciplinary team to meet patient needs.
  • Complete face-to-face visits, assess eligibility, and provide clinical information to the medical director for recertification.
  • Communicate effectively with patients, families, and healthcare practitioners to ensure seamless care coordination.
Requirements:
  • Master's degree in Nursing with a focus on Nurse Practitioner education.
  • Current nursing licensure and CPR certification.
  • Minimum one year of Nurse Practitioner experience and one year of hospice or palliative care experience.
  • Certification in a specialist area, such as Hospice and Palliative Nursing (CHPN), is preferred.
  • Excellent observation, verbal, and written communication skills, problem-solving skills, and nursing skills.
