West River Care is seeking a highly skilled and compassionate Hospice Nurse Practitioner to join our team. As a key member of our interdisciplinary team, you will provide comprehensive hospice services to patients and their families, promoting comfort, dignity, and quality of life.
Key Responsibilities:- Provide medical care and management of terminal illnesses, ensuring compliance with state Nurse Practice Act and collaborating with physicians.
- Evaluate patient health history, perform physical assessments, and develop individualized plans of care.
- Prescribe medications, coordinate care services, and collaborate with the interdisciplinary team to meet patient needs.
- Complete face-to-face visits, assess eligibility, and provide clinical information to the medical director for recertification.
- Communicate effectively with patients, families, and healthcare practitioners to ensure seamless care coordination.
- Master's degree in Nursing with a focus on Nurse Practitioner education.
- Current nursing licensure and CPR certification.
- Minimum one year of Nurse Practitioner experience and one year of hospice or palliative care experience.
- Certification in a specialist area, such as Hospice and Palliative Nursing (CHPN), is preferred.
- Excellent observation, verbal, and written communication skills, problem-solving skills, and nursing skills.
