About the Role:

We are seeking a skilled and compassionate Social Worker (MSW) to join our interdisciplinary team at Heartland Hospice. As a key member of our team, you will provide psychosocial support to patients and their families, utilizing your expertise to identify needs, goals, and interventions that align with our established plan of care.

Responsibilities:

  • Provide psychosocial support to patients and their families, assessing their needs and developing interventions to address them.
  • Collaborate with the interdisciplinary team to ensure comprehensive care and support for patients and their families.
  • Develop and implement individualized plans of care that address the unique needs and goals of each patient and family.
  • Provide education and support to patients and their families on end-of-life care, hospice services, and related topics.
  • Document patient interactions, assessments, and interventions in accordance with our policies and procedures.

Requirements:

  • Master's Degree in Social Work from a CSWE-accredited school.
  • Licensure as required by the state in which the hospice is located.
  • One year of experience as a Social Worker in a healthcare and/or hospice setting, with three to five years preferred.
  • Knowledge of terminally ill patients and their families, as well as understanding of hospice and the psychosocial dynamics of illness, loss, and death.
  • Ability to apply knowledge of the special needs of hospice patients and families, and to support others at a time of crisis.
