We are seeking a skilled Medical Social Worker to join our team at Washington Hospital. As a Medical Social Worker, you will play a critical role in providing psychosocial assessments and interventions to patients and their families, working closely with the RN Case Manager to ensure seamless care coordination.
Key Responsibilities- Conduct thorough psychosocial assessments to identify patient needs and develop effective intervention plans
- Collaborate with the RN Case Manager to coordinate patient care and ensure timely discharges
- Develop and maintain knowledge of community resources and referrals to support patient needs
- Provide education and support to patients and families on coping and decision-making strategies
- Participate in patient and family conferences as needed
- Manage behavioral emergencies and provide crisis intervention as needed
- Masters in Social Work required
- California LCSW required
- Three to five years of work experience in a healthcare setting
- Acute hospital experience preferred
- Knowledge of post-acute care community resources and referrals
- Basic computer skills required
