About the Role

The Ambulatory Care Management Department at Corewell Health is comprised of skilled professionals dedicated to delivering exceptional patient care. As a Social Worker, you will play a vital role in identifying and connecting with patients who require behavioral health support or chronic medical management. Your expertise will focus on developing healthy behaviors and coping skills to improve overall health outcomes.

Working closely with primary care providers, you will partner to develop tailored treatment approaches for patients experiencing chronic disease and/or behavioral health concerns. Your comprehensive transitions of care support will ensure seamless patient care, preventing readmissions and reducing healthcare costs.

As a member of our Ambulatory Care Management team, you will also provide resources and support for patients facing barriers related to Social Determinants of Health (SDoH). Your dedication to patient-centered care will make a meaningful difference in the lives of those we serve.

About Our Outpatient Clinics

Corewell Health operates over 300 outpatient clinics across Michigan, offering a diverse range of care options to our communities. With more than 40 different types of primary care and specialty practices, you will have the opportunity to learn and grow in a dynamic environment.

Scope of Work

As a Social Worker, your key responsibilities will include:

  • Assessing patient needs and developing personalized care plans
  • Providing peer support and role modeling to promote healthy behaviors and coping skills
  • Collaborating with primary care providers to develop effective treatment approaches
  • Providing comprehensive transitions of care support to ensure seamless patient care
  • Identifying and addressing barriers related to Social Determinants of Health (SDoH)
