Job Summary:

The Medical Social Worker - Patient Care Specialist will provide direct case management services to patients, supporting them and their families/caregivers in dealing with social, emotional, and practical aspects necessary for maintaining independence.

Key Responsibilities:
  • Exercise independent judgment and discretion in managing a caseload of patients.
  • Perform initial and ongoing psychosocial assessments to develop, implement, and update tailored care plans and interventions.
  • Collaborate with caregivers and support systems in care planning and service delivery.
  • Identify and address risks and other psychosocial issues.
  • Determine each patient's service needs; advocate, refer, coordinate, and follow up promptly.
  • Provide individual counseling and facilitate family meetings.
  • Engage with broader community resources to fulfill specific patient needs.
  • Conduct discharge planning upon a patient's exit from the program.
  • Lead patient and/or caregiver groups and contribute to events like weekend caregiver workshops.
Requirements:
  • Master's degree in Social Work from an accredited institution.
  • At least one year of experience working with the frail or elderly population.
  • Capable of serving a diverse patient base.
  • Proficient in interdisciplinary team collaboration.
  • Excellent interpersonal, organizational, and follow-up skills.
  • Strong written and verbal communication abilities.
  • Competence in Microsoft Office and general computer literacy.
  • Bilingual skills may be needed for specific roles.
