Scope of Work

We are seeking a skilled Counselor Specialist to join our team in Fremont. In this role, you will plan and prioritize care for individuals and populations, focusing on strategies that promote optimal health. Your expertise will involve providing peer support, role modeling, and developing life skills and health/wellness to individuals and families during home visits. You will also assess internal referrals, provide individual therapy services, and monitor patient progress.

Key Responsibilities

  • Develop and implement plans of care, making recommendations to primary care physicians, specialists, and other healthcare team members.
  • Assess and provide follow-up for the Systematic Case Review process, collaborating with psychiatrists and serving as a liaison between primary care physicians and consulting psychiatrists.
  • Respond to patients' urgent mental health concerns, including suicide risk assessments, and assist patients in accessing appropriate levels of care.

Requirements

To be successful in this role, you will need:

  • 2 years of relevant experience in the field.
  • LMSW (Master Social Worker) or LLMSW (Limited Licensed Master Social Worker) in Michigan.

This position is located at our Fremont Medical Office Building, 230 W Oak St, Fremont, MI.
