Job Summary

We are seeking a highly skilled Care Coordinator to join our team at Ascentria Care Alliance. As a Care Coordinator, you will be responsible for providing direct support, advocacy, and oversight to individuals served according to their treatment plans.

Your primary goal will be to assess, evaluate, and develop a plan of care consistent with the needs of the individual. You will establish links within the community to include making referrals, coordination, and advocacy to facilitate access by the individual to the services and supports identified by the individual within his/her assessment and Individualized Service Plan (ISP).

Key Responsibilities:

  • Provides direct support, advocacy, and oversight to individuals served according to their treatment plans.
  • Assesses, evaluates, and develops a plan of care consistent with the needs of the individual.
  • Establishes links within the community to include making referrals, coordination, and advocacy to facilitate access by the individual to the services and supports identified by the individual within his/her assessment and Individualized Service Plan (ISP).
  • Develops strong and effective working relationships with external and internal stakeholders, including, but not limited to public/private guardians, provider agencies, and community vendors.
  • Conducts job responsibilities at all times in a way that respects the rights, privacy, and dignity of the people served.
  • Ensures documentation standards, site standards, and services delivered meet and/or exceed contractual regulatory and agency obligations.
  • Meets all documentation deadlines, including but not limited to: ISP, assessments, census, continuing stay reviews, etc.

Requirements:

  • High School Diploma or GED is required, related associate's degree preferred.
  • 6 months of related experience is desired.
  • Valid State-issued Driver's License and proof of current insurance required.
  • MHRT-C or MHRT-C Provisional and maintain with 18 approved CEU's every 2 years.
