Job Description

Family and Children's Agency (FCA) is a nonprofit human service organization dedicated to enhancing the social and emotional well-being of our clients. Our Agency delivers services to over 13,000 people each year, offering more than 30 programs that cater to diverse needs across all ages. We partner with individuals, families, organizations, and communities to build better lives.

Position Summary:

The MOMS Home Visitor performs regular home visits for families with children aged birth to eight years old. These visits focus on child development, strengthening parenting skills, and promoting a positive parent-child relationship. The home visitor also provides case management to families as needed and connects them with referrals to other community resources.

Key Responsibilities:

Provide home visiting and case management services to a caseload of 10 to 12 families in the Greater Norwalk Area.Facilitate groups and workshops as assigned, and assist with transportation and/or childcare as needed.Outreach to identify and recruit new clients for the MOMS Program.Connect clients with appropriate referrals to community resources, including health and social services.Accompany clients to medical and agency appointments as necessary.Serve as an advocate for clients with schools, healthcare providers, and agency staff.Provide accurate and relevant information on child development and child health and wellness.Identify client goals and work towards their attainment.Assist clients in strengthening parenting skills and problem-solving abilities.Help clients identify and utilize their informal support networks.Participate in orientation and training sessions.Attend regularly scheduled supervision and staff meetings.Attend community and state-wide meetings and trainings as assigned.Complete all assigned paperwork in an accurate and timely manner.Maintain client records in our online database.

Requirements:

  • Awareness of cultural and socioeconomic characteristics of clients served.
  • Bilingual – Spanish/English.
  • A valid driver's license and reliable transportation.
  • Knowledge and expertise in child development, parenting, community resources, outreach, prenatal/postpartum healthcare, and diverse populations.
  • A Bachelor's degree preferred; Associate's degree in human services, social work, early childhood education, or a related field required.
  • Experience facilitating groups preferred.
