Position Overview: Ambulatory Care Clinical Pharmacist

Key Responsibilities:

  • Collaborate with healthcare providers and clinical teams to deliver superior patient care.
  • Conduct comprehensive medication therapy management and provide patient education.
  • Advocate for high-quality, evidence-based, and cost-efficient pharmaceutical care.
  • Engage in the training and mentoring of pharmacy residents and students.
  • Participate in pharmacy committees and governance initiatives.
  • Contribute to continuous improvement efforts within the pharmacy department.
  • Support the educational advancement of the department and the broader health system.
  • Communicate effectively with patients and colleagues, both verbally and in writing.
  • Demonstrate proficiency with relevant pharmacy software and equipment.

Qualifications:

  • Education: A Bachelor's Degree in Pharmacy is required; a Doctorate of Pharmacy is preferred.
  • Experience: A minimum of one year in a postgraduate residency or two years as a registered pharmacist in a health system pharmacy is required. Experience in an ambulatory or clinic setting is highly valued.
  • Licensure/Certification: Must possess current pharmacy licensure in the relevant state(s) within 30 days of employment. Board of Pharmacy Specialties Certification or equivalent must be obtained within two years of employment.
  • Additional Skills: Ability to function effectively as part of a high-performance team and willingness to travel to various facilities as needed.
