Ambulatory Care - Clinical Pharmacist (Primary Care)

Key Responsibilities:

  • Collaborate with healthcare providers and clinical personnel to deliver superior patient care.
  • Conduct comprehensive medication therapy management and provide patient counseling.
  • Advocate for high-quality, evidence-based, and cost-efficient pharmaceutical care.
  • Engage in the education of residents and students.
  • Participate in pharmacy committees and governance initiatives.

  • Demonstrate proficiency with relevant technology and software.

Qualifications:

  • Education: A Bachelor's Degree in Pharmacy is required; a Doctorate of Pharmacy is preferred.
  • Experience: A minimum of one year in a postgraduate residency or two years as a registered pharmacist in a health system pharmacy is required. Experience in an ambulatory or clinic setting is highly desirable.
  • Licensure/Certification: Must obtain current Pharmacy licensure in the relevant state(s) within 30 days of starting. Board of Pharmacy Specialties Certification or equivalent is required within two years of starting.
  • Additional Skills: Ability to work collaboratively within a high-performance team and travel to various facilities as necessary.
