Job Summary:

Key Responsibilities:

  • Provide direct patient care in an ambulatory setting, including infusion therapy and home health agency experience.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
  • Conduct thorough assessments, identify potential health risks, and develop strategies to mitigate them.
  • Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal care.

Requirements:

  • Current RN license in the appropriate state.
  • Minimum of one year clinical experience in infusion therapy and/or home health agency.
  • High School Diploma or GED, required. 4 Year / Bachelors Degree, preferred.
  • Graduate of an accredited school of nursing required.
  • Additional Skills: Appropriate customer service, effective and appropriate customer relations, verbal and non-verbal communication techniques, interpersonal relationship skills, conflict resolution, critical thinking, and computer literacy.
