Leoforce is seeking a highly skilled Registered Nurse Ambulatory Specialty to join our team. As an Ambulatory Care Nurse Specialist, you will be responsible for providing high-quality patient care in an outpatient setting.
Key Responsibilities:
- Provide direct patient care in an ambulatory setting, including infusion therapy and home health agency experience.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Conduct thorough assessments, identify potential health risks, and develop strategies to mitigate them.
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal care.
Requirements:
- Current RN license in the appropriate state.
- Minimum of one year clinical experience in infusion therapy and/or home health agency.
- High School Diploma or GED, required. 4 Year / Bachelors Degree, preferred.
- Graduate of an accredited school of nursing required.
- Additional Skills: Appropriate customer service, effective and appropriate customer relations, verbal and non-verbal communication techniques, interpersonal relationship skills, conflict resolution, critical thinking, and computer literacy.
