We are seeking a highly skilled and experienced Mental Health Care Specialist to join our team at the Cabarrus Rowan Community Health Center.

About the Role

As a Mental Health Care Specialist, you will work closely with our patients to assess their mental health needs, develop personalized treatment plans, and provide ongoing support and guidance. You will also collaborate with other healthcare professionals to ensure seamless care coordination and implementation of treatment plans.

Responsibilities
  • Conduct thorough assessments of patients' mental health needs, including interviews, observations, and review of medical history.
  • Develop and implement individualized treatment plans that address patients' specific mental health needs, including medication management and counseling services.
  • Collaborate with other healthcare professionals to ensure effective care coordination and implementation of treatment plans.
  • Provide ongoing support and guidance to patients and their families, addressing any questions or concerns they may have.

Salary: $90,000 - $115,000 per year, depending on experience.
