Job Summary: Community Mental Health Coordinator

About the Role:
Premier Services of Carolina, Inc. is seeking a compassionate and organized Community Mental Health Coordinator to join our team. As a key member of our community-based rehabilitation team, you will be responsible for providing direct support to individuals with mental health and substance abuse issues.

Key Responsibilities:
- Provides intensive case management to individuals with mental health and substance abuse diagnoses
- Linkage and referral to formal and informal supports
- Monitoring and follow-up on the status of beneficiaries' progress and the effectiveness of strategies and interventions

Requirements:
- A licensed substance abuse professional with a minimum of one year of experience working with adults with mental health and substance abuse diagnoses
- Excellent communication and documentation skills
- Ability to maintain strict confidentiality
- Valid driver's license and reliable transportation
