Position Title: Family Support Team Case Manager

Key Responsibilities:

  • Deliver comprehensive case management services to a variety of families;
  • Assist families throughout forensic interview processes;
  • Engage clients in available services and support systems;
  • Collaborate with community professionals to enhance service delivery;
  • Design and oversee individualized service plans;
  • Lead multi-disciplinary team discussions;
  • Maintain accurate documentation and adhere to reporting standards;
  • Ensure adherence to evidence-based practices in all services provided.

Required Qualifications: A Bachelor’s degree along with relevant experience is essential. Proficiency in both Spanish and English is required.
