Case Manager for Parent-Child Assistance Program

Title: Parent-Child Assistance Program Case Manager
Group: G_Apprentice
Supervised by: Program Manager
Location: Skagit County
Starting Wage: $19.00/hour
Objective: To deliver high-quality services and manage complex tasks effectively. This role involves implementing strategies that enhance the likelihood of successful outcomes for clients. Responsibilities may include providing guidance to others, demonstrating excellent communication and prioritization skills, and fostering self-initiative. The position requires moderate to high levels of cooperation with both internal and external stakeholders.
The Parent-Child Assistance Program (PCAP) is a home visitation case management initiative designed for high-risk mothers who have a history of alcohol and/or drug use during pregnancy.
Key Responsibilities:
- Maintain a caseload of 15-20 families.
- Conduct home and community visits with clients approximately twice a month.
- Assist clients in establishing goals based on their individual assessments and guide them in achieving these goals while monitoring their progress.
- Identify and engage with community providers and agencies relevant to client needs, assisting them in navigating these systems.
- Input data on client progress both monthly and bi-annually.
- Participate in bi-weekly individual clinical supervisions and weekly team meetings.
- Keep current certifications in CPR, Infant CPR, and First Aid.
- Work independently and effectively in the field with minimal supervision.
Qualifications: A Bachelor's degree along with one year of experience in community-based work with high-risk populations is required. Candidates must possess strong organizational and time management skills, as well as the ability to conduct sensitive and confidential interviews. Experience with individuals facing substance abuse challenges is preferred. A valid driver's license is necessary.
Benefits: Brigid Collins offers eligible employees comprehensive medical, dental, and vision insurance. After one year of employment, employees working 20 or more hours per week will receive an employer contribution to a 403(B) retirement account.
