Our senior patients in the communities we serve encounter various social challenges, known as "Social Determinants of Health" (SDOH).
Referred to internally as the "Gather Guide Partner," this role involves working closely with our patients to assess their needs, exploring solutions to overcome barriers, and ultimately facilitating access to essential resources.
These challenges may include issues such as food insecurity, social isolation, and transportation difficulties.The Gather Guide Partner (GGP) will develop a deep understanding of our patients and their unique circumstances, becoming their trusted and consistent point of contact.
The GGP will collaborate with our primary care providers, Emergency Medical Technician Partners (EMTs), and clinic administrators as part of a cohesive care team.
As a result, the GGP plays a crucial role in our mission to deliver the highest quality of care and support, driven by a profound sense of commitment and compassion, ultimately striving for equity in healthcare.
Daily Responsibilities
The daily routine of a GGP will vary based on the needs of our patients.
The majority of your time will be spent in your assigned clinic, where you will engage with our patients in person, as well as through phone calls and virtual consultations.
Some of the daily responsibilities will include, but are not limited to:- Managing phone communications (both outbound and inbound) with patients to provide tailored and high-quality service
- Conducting in-person assessments with new patients to identify their specific social needs and documenting findings
- Escorting patients to examination rooms for clinician consultations
- Collaborating with patients and the care team to create individualized care plans
- Researching community resources relevant to our patients' needs
- Proactively engaging with community organizations to explore potential partnership opportunities
- Continuously expanding our resource network to address gaps in transportation, food access, activities, and social connections
- At least one year of experience in medical settings, community organizations, hospitality, or related service fields
- Familiarity with medical terminology
- Experience in one-on-one interactions with patients or customers
- Proven experience in roles requiring frequent communication via phone and in-person
- High school diploma or G.E.D.
- Proficiency with basic office tools such as email and calendar applications
- Strong verbal, written, and reading comprehension skills in English
- Availability to work standard office hours, Monday through Friday
- Eligibility to work for any employer in the United States
- Experience working with seniors or individuals facing complex social challenges
- Background in a primary care setting
- Certification as a phlebotomist
- Associate's or bachelor's degree
- Bilingual proficiency in Mandarin, Cantonese, or Vietnamese
- Experience as a birth or end-of-life doula
