RN - Home Health Case Manager - FT Days - St. Peter Home Health

We are seeking a skilled and compassionate RN Case Manager to join our Home Health team at Good Samaritan Society. As a key member of our care coordination team, you will work closely with patients, families, and healthcare providers to develop and implement personalized care plans that promote optimal health and well-being.
Responsibilities- Conduct comprehensive assessments of patients' physical, emotional, and social needs
- Develop and implement individualized care plans that address patients' unique needs and goals
- Collaborate with healthcare providers, family members, and caregivers to ensure seamless care coordination
- Monitor patients' progress and adjust care plans as needed
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making
- Bachelor's degree in nursing (BSN) preferred
- Minimum of one to two years of professional nursing experience in a clinic care setting
- Current RN licensure with the State Board of Nursing where the practice of nursing is occurring
- Ability to work independently and as part of a team
- Strong communication and interpersonal skills
