Job Summary:

The Cardio Metabolic Clinic (CMC) is seeking a skilled Registered Nurse to provide professional nursing care and medical care prescribed by licensed healthcare providers. As an RN Coordinator, you will play a critical role in facilitating care coordination of patients to ensure positive outcomes and effective overall plans of care.

About the Role:

This is a full-time opportunity with an estimated salary range of $65,000 - $80,000 per year, depending on experience. The ideal candidate will have a minimum of three years of relevant nursing experience, preferably in clinic and cardiovascular nursing. A Bachelor of Science in Nursing (BSN) degree is preferred.

Key Responsibilities:

  • Collaborate with physicians from multi-specialties to develop referrals, coordinate clinic and outpatient visits, and navigate clinical care
  • Provide procedural coordination for applicable BH Program: LAAO, Structural Heart Program, etc., and other related procedures or diagnostic testing
  • Receives physician referrals and compiles clinical data for presentation to a multi-specialty physician team at scheduled program meetings
  • Perform clinical and social-economical assessment of referred patients and navigate care between physicians and diagnostic services
  • Educate patient and family about tests/procedures, results, medications, nutrition, and other health-related issues
