As a Clinical Care Specialist - Home Infusion at Optum, you will play a vital role in ensuring the safe and effective delivery of infusion therapy to home-based patients. This position requires strong communication and interpersonal skills, as well as the ability to work collaboratively with patients, families, and healthcare providers.

About the Role:

  • Establish and maintain positive relationships with patients and their caregivers to provide comprehensive care and support.
  • Assess patient appropriateness for home infusion therapy, including safety evaluations and documentation.
  • Obtain and document patient medical history, particularly as related to the diagnosis being treated with prescribed infusion therapy.
  • Complete all required clinical documentation thoroughly and timely, including nursing plans of care and visit notes.
  • Participate in case conferences to comprehensively plan patient care with the interdisciplinary team.

Salary and Benefits:

We offer a competitive salary range of $90,000 - $130,000 per year, based on experience and location. In addition to this, we provide excellent benefits, opportunities for professional growth and development, and a dynamic work environment. The estimated salary range includes an 18% bonus, reflecting the company's commitment to rewarding its employees for their hard work and dedication.
