Key Responsibilities of the Social Service Director:

The Social Service Director (SSD) plays a crucial role in addressing the psychosocial needs of our residents. Key tasks include:

  • Conducting comprehensive Social Service assessments, including MDS, psychosocial, and trauma evaluations.
  • Collaborating with residents, their families, and an interdisciplinary healthcare team to develop personalized care plans.
  • Encouraging resident involvement in their care plans and assisting them in setting realistic, achievable goals.
  • Reporting any concerns to the Administrator.
  • Managing resident grievances effectively.
  • Providing support to residents and families dealing with age-related illnesses, grief, and transitions to new living arrangements.
  • Coordinating discharge planning and community service connections.
  • Assisting residents with Advance Directives and legal documentation while maintaining accurate records for HCP, Guardians, and Rogers reviews.
  • Utilizing resources to help residents achieve or maintain optimal physical, mental, and psychosocial well-being.
  • Engaging with residents daily to offer opportunities for expressing concerns and reassuring them of our commitment to their welfare.

Qualifications:

  • Preferred experience in long-term care nursing facilities.
  • A degree in Social Services is required.
