Key Responsibilities of the Social Service Director:
The Social Service Director (SSD) plays a crucial role in addressing the psychosocial needs of our residents. Key tasks include:
- Conducting comprehensive Social Service assessments, including MDS, psychosocial, and trauma evaluations.
- Collaborating with residents, their families, and an interdisciplinary healthcare team to develop personalized care plans.
- Encouraging resident involvement in their care plans and assisting them in setting realistic, achievable goals.
- Reporting any concerns to the Administrator.
- Managing resident grievances effectively.
- Providing support to residents and families dealing with age-related illnesses, grief, and transitions to new living arrangements.
- Coordinating discharge planning and community service connections.
- Assisting residents with Advance Directives and legal documentation while maintaining accurate records for HCP, Guardians, and Rogers reviews.
- Utilizing resources to help residents achieve or maintain optimal physical, mental, and psychosocial well-being.
- Engaging with residents daily to offer opportunities for expressing concerns and reassuring them of our commitment to their welfare.
Qualifications:
- Preferred experience in long-term care nursing facilities.
- A degree in Social Services is required.
