Clinical Social Worker LCSW

We are seeking a skilled Clinical Social Worker LCSW to join our team at UMass Memorial Health Care. As a Clinical Social Worker LCSW, you will play a vital role in providing exceptional patient care and support to our patients and their families.
Job Summary:The Clinical Social Worker LCSW will be responsible for assessing patients' and families' needs, providing counseling and therapy, and developing discharge plans to ensure a smooth transition to home or other care settings. You will work closely with our multidisciplinary team to identify and address barriers to discharge and treatment, and advocate for patients and families as they navigate the healthcare system.
Responsibilities:- Conduct biopsychosocial assessments of patients and families to identify their needs and develop individualized treatment plans.
- Provide counseling and therapy to patients and families to address their emotional and psychological needs.
- Develop and implement discharge plans to ensure a smooth transition to home or other care settings.
- Collaborate with our multidisciplinary team to identify and address barriers to discharge and treatment.
- Advocate for patients and families as they navigate the healthcare system.
- Master's degree in Social Work from an accredited program.
- Current Massachusetts Licensed Clinical Social Worker (LCSW) licensure.
- One to three years of experience in social work, preferably in a healthcare setting.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
