Pediatric Primary Care Social Worker

We are seeking a skilled and compassionate Pediatric Primary Care Social Worker to join our team at UMass Memorial Health. As a key member of our multidisciplinary team, you will provide short-term counseling services to children and families, working closely with clinic providers and staff to ensure seamless care.

Key Responsibilities:

  • Conduct psychosocial evaluations and assessments to identify patient needs and develop effective treatment plans.
  • Provide individual and group therapy, as well as education and support to patients and families.
  • Collaborate with healthcare providers to coordinate care and ensure continuity of services.
  • Document all interactions and interventions in the medical record, ensuring accurate and timely billing.
  • Develop and maintain relationships with community resources and agencies to provide patients with access to necessary services.

Requirements:

  • Masters degree in Social Work.
  • Current Massachusetts Licensed Clinical Social Worker (LICSW) licensure.
  • One to three years of social work experience, with a focus on pediatric care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
