Position Overview:

The Non-Medical Case Manager (NMCM) plays a vital role in enhancing access to and retention of essential medical and support services aimed at overcoming obstacles to HIV care. This position is accountable for determining eligibility, facilitating enrollment and re-enrollment in the Ryan White program, and evaluating client requirements for Medical Case Management.

Key Responsibilities:
  • Assess eligibility for the Ryan White grant program and manage annual renewals
  • Conduct client assessments and facilitate enrollment into grant-supported services
  • Provide assistance to clients in accessing referrals and community resources
  • Create detailed service plans tailored to individual client needs
  • Gather and input data into the CAREWare system
  • Engage in Quality Management and Improvement initiatives
Essential Qualifications:
  • A minimum of 3 years of experience in Case Management within the healthcare sector
  • Familiarity with Medicare, Medicaid, and various assistance programs
  • Knowledge of HIPAA regulations and privacy practices
  • Exceptional customer service and interpersonal abilities
  • Strong organizational and time management skills
  • Capability to work both independently and collaboratively
  • Proficiency in Microsoft Office and data entry
  • Awareness of health information sensitivities and HIV/AIDS-related matters
  • Outstanding communication skills
  • Cultural competence in engaging with diverse populations
Desirable Qualifications:
  • Experience and knowledge of the Ryan White grant program
