Position Overview:

The Non-Medical Case Manager (NMCM) plays a crucial role in facilitating access to essential medical and support services aimed at overcoming barriers to HIV care. This position is tasked with managing eligibility assessments, enrollment processes, and providing assistance in determining qualifications for various benefits.

Key Responsibilities:
Conduct initial eligibility screenings for the Ryan White grant program and manage annual renewals. Perform comprehensive client assessments and facilitate enrollment into grant-supported services. Provide clients with referrals and address service needs by connecting them with internal and external community resources. Develop detailed, individualized service plans tailored to client needs. Collect and input data into the CAREWare data management system, adhering to established policies and timelines. Engage in Quality Management and Quality Improvement initiatives as required. Minimum Qualifications:
A minimum of three years of experience in case management within a healthcare setting. Familiarity with Medicare, Medicaid, and other federally or state-funded assistance programs. Understanding of HIPAA regulations and the ability to maintain privacy and confidentiality. Strong customer service and interpersonal communication skills. Excellent organizational and time management abilities. Proven ability to work collaboratively in a team environment. Capability to work independently with minimal supervision. Proficiency in Microsoft Office and data entry applications. Sensitivity to confidential health information and issues related to HIV/AIDS. Exceptional oral and written communication skills. Cultural competence in engaging with diverse populations. Preferred Qualifications:
Experience and knowledge of the Ryan White grant program.
