Position Overview:

The Non-Medical Case Manager (NMCM) plays a crucial role in enhancing access to and retention of essential medical and support services aimed at overcoming obstacles to HIV care. This position is responsible for determining eligibility, facilitating enrollment and re-enrollment in the Ryan White program, and evaluating client requirements for Medical Case Management.

Key Responsibilities:
  • Assess eligibility for the Ryan White grant program and manage annual renewals
  • Conduct client assessments and facilitate enrollment into grant-supported services
  • Provide clients with referrals and connect them to community resources
  • Create detailed service plans tailored to client needs
  • Gather and input data into the CAREWare system
  • Engage in Quality Management and Improvement initiatives
Essential Qualifications:
  • A minimum of 3 years of experience in Case Management within a healthcare setting
  • Familiarity with Medicare, Medicaid, and various assistance programs
  • Comprehensive understanding of HIPAA regulations and privacy standards
  • Strong interpersonal and customer service abilities
  • Excellent organizational and time management skills
  • Capability to work both independently and collaboratively
  • Proficient in MS Office and data entry tasks
  • Awareness of health information sensitivities and HIV/AIDS issues
  • Outstanding communication skills
  • Cultural competence in engaging with diverse populations
Desirable Qualifications:
  • Experience and knowledge of the Ryan White grant program
