As a Physical Therapist with BAYADA Home Health Care, you will be an integral member of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing to clients in the comfort of their own homes. With a focus on patient-centered care, you will work closely with clients, families, and healthcare professionals to develop and implement individualized care plans that promote independence, safety, and overall well-being.

Responsibilities include:

  • Conducting home visits to assess client needs and develop personalized care plans
  • Implementing evidence-based interventions, such as manual therapeutic exercises, gait training, and balance assessments
  • Collaborating with nursing and other healthcare professionals to ensure seamless care coordination
  • Providing education and instruction to clients and families on rehabilitative care and activities necessary for independent living
  • Accurately documenting client observations, interventions, and evaluations

Qualifications:

  • Current Pennsylvania Physical Therapist license
  • Minimum one year of recent work experience as a Physical Therapist
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
  • Ability to work independently and manage time effectively
  • Strong interpersonal, organizational, and problem-solving skills
