Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled Home Health Physical Therapist to join our team in Ridley Park, Pennsylvania.

Job Summary:

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our adult and geriatric clients in their homes. As a Home Health Physical Therapist, you will be an integral part of a multidisciplinary healthcare team, working closely with nurses and other healthcare professionals to develop and implement individualized care plans.

Responsibilities:
  • Conduct home visits to assess clients' physical needs and develop personalized care plans.
  • Perform diagnostic tests and measurements to evaluate clients' mobility, strength, and balance.
  • Implement evidence-based interventions, such as manual therapeutic exercises and gait training, to promote clients' independence and mobility.
  • Collaborate with the healthcare team to revise and update care plans as needed.
  • Educate clients and their families on rehabilitative care and activities to promote healthy living.
Requirements:
  • A current Pennsylvania Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
