Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled Home Health Physical Therapist to join our team in Ridley Park, Pennsylvania.

Job Summary:

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our clients in the comfort of their own homes. As a Home Health Physical Therapist, you will work closely with our multidisciplinary team to develop and implement individualized care plans that promote independence, mobility, and overall well-being.

Responsibilities:
  • Conduct thorough assessments and evaluations to identify client needs and develop personalized care plans.
  • Implement evidence-based interventions, including manual therapeutic exercises, gait training, and balance activities, to improve client mobility and function.
  • Collaborate with the healthcare team to ensure seamless care coordination and communication.
  • Provide education and training to clients and their families on proper techniques and strategies for maintaining independence and preventing complications.
  • Document client progress and outcomes accurately and thoroughly, utilizing electronic medical records.
Requirements:
  • Current Pennsylvania Physical Therapist license.
  • Minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
