Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled Home Health Physical Therapist to join our team in Prospect Park, Pennsylvania.

Job Summary:

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our clients in their homes. As a Home Health Physical Therapist, you will work closely with our multidisciplinary team to develop and implement individualized care plans that promote the health, safety, and independent living of our clients.

Responsibilities:
  • Conduct home visits to assess and treat clients with physical therapy needs.
  • Develop and implement individualized care plans that address the client's physical therapy needs.
  • Provide manual therapeutic exercises, gait training, balance, and other interventions to promote client mobility and independence.
  • Continuously assess and revise the care plan to ensure the client's needs are met.
  • Educate and instruct clients, family members, or other client representatives on rehabilitative care and activities necessary to promote the client's health and safety.
  • Accurately document observations, interventions, and evaluations pertaining to client care management and services provided.
Qualifications:
  • A current Pennsylvania Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
