Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled Home Health Physical Therapist to join our team in Prospect Park, Pennsylvania.

Job Summary:

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our clients in the comfort of their own homes. As a Home Health Physical Therapist, you will work closely with our multidisciplinary team to develop and implement individualized care plans that promote the health, safety, and independent living of our clients.

Responsibilities:
  • Conduct home visits to assess clients' physical needs and develop personalized care plans.
  • Implement evidence-based interventions, such as manual therapeutic exercises, gait training, and balance assessments, to promote clients' physical function and mobility.
  • Collaborate with our nursing team to ensure seamless care coordination and communication.
  • Educate clients and their families on rehabilitative care and activities necessary to promote their health and safety.
  • Accurately document client care management and services provided, utilizing our state-of-the-art touch pad tablet.
Qualifications:
  • A current Pennsylvania Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
