Job Summary

We are seeking a highly motivated and organized individual to join our team as a Transition Coordinator for Vermont Pathways to Partnership. In this role, you will work closely with students with disabilities, their families, and local schools to coordinate necessary services and support their transition to post-secondary education and employment.

Key Responsibilities
  • Coordinate services with students with disabilities and their families to ensure a smooth transition to post-secondary education and employment.
  • Collect and present data to inform program development and outcomes.
  • Provide technical assistance to local education agencies to support the transition process.
  • Ensure budgets and task details are meticulously organized to ensure efficient program delivery.
  • Cultivate and maintain relationships with contacts at regional schools to ensure effective communication and collaboration.
Requirements
  • Two or more years of experience working in a human services field.
  • Associates degree or higher in human services, psychology, social work, or special education.
  • Experience working with disabled youth.
