Position Overview

Role: The Community-Based Service Coordinator plays a vital role within an interdisciplinary team, delivering flexible and compassionate support services aimed at fostering stability, recovery, and reintegration for individuals facing mental health challenges and histories of homelessness.

Organization: Pathways Vermont is a progressive nonprofit organization dedicated to the mission of eradicating homelessness and providing innovative mental health services through continuous supportive social work.

Key Responsibilities:

  • Establish and nurture trusting relationships with clients, employing a client-centered, harm reduction, and strengths-based approach.
  • Offer personalized support and guidance on essential life skills, housing issues, health maintenance, and financial management.
  • Assist clients in navigating housing options, including unit viewings, moving assistance, and resolving landlord-client disputes.
  • Educate and advocate for clients regarding their benefits and entitlements, such as Social Security and public assistance programs.
  • Maintain accurate written and electronic records, compile reports, and complete necessary program documentation.
  • Participate in annual staff retreats and training sessions, contributing to team cohesion and professional development.
  • Perform additional duties as assigned to support the overall mission of the organization.

Qualifications:

  • Experience working within a Housing First framework and a solid understanding of the challenges faced by individuals with mental health or substance use issues.
  • Strong communication, writing, and computer skills.
  • Ability to manage time effectively, prioritize tasks independently, and meet documentation requirements.
  • Possession of a valid driver's license, reliable vehicle, and current automobile insurance.
  • Capability to lift and carry up to 50 pounds.
  • Willingness to engage in self-reflection and adapt interpersonal approaches.
  • Comfort with risk and uncertainty in a dynamic work environment.

  • Hourly wage ranging from $20 to $21.
