Position Overview

The Patient Transition Coordinator is responsible for overseeing and managing the discharge planning process for individuals moving from our healthcare facility to their homes or alternative care environments.

Key Responsibilities:
  • Coordinate with patients, families, healthcare teams, Utilization Review personnel, and community resources to facilitate a smooth transition and ensure access to essential support services.
  • Assess patient needs and create tailored discharge plans that address individual circumstances.
  • Maintain knowledge of federal, state, and local regulations related to healthcare and case management.
  • Utilize Electronic Medical Records to document patient information and discharge plans effectively.
  • Provide compassionate support to patients and families during the transition process, demonstrating empathy and understanding.
Qualifications:
  • Associate degree in social work, nursing, or a related discipline.
  • A minimum of one (1) year of experience in various clinical environments.
  • Experience in case management or discharge planning is preferred.
  • Familiarity with medical terminology and healthcare systems is essential.
  • Strong communication and interpersonal skills to work collaboratively with patients and healthcare professionals.
  • Proficient in computer applications, including electronic health records and Microsoft Office.
  • Ability to navigate complex healthcare systems and manage time effectively.
