The RN Care Manager is a critical member of our care team, working closely with complex renal patients in their homes, dialysis centers, and virtually as needed.

The primary focus will be to improve patient outcomes by helping patients get permanent access, promoting home dialysis modalities & kidney transplantation, educating patients on self-management, addressing risks associated with comorbid conditions, and coordinating their care.

Working in a hybrid telehealth environment, you will conduct comprehensive assessments, identify gaps in care, and create and implement care plans that address identified needs and improve patient health.

You will also provide clinical oversight to non-licensed support teams and delegate tasks as appropriate.

Requirements:

  • 1+ years of nursing experience in case management or care management
  • ~2+ years of healthcare-related experience
  • Current, valid BLS certification
  • Core values consistent with a patient-centered approach to care
  • Strong community engagement and facilitation skills
  • Ability to travel throughout the assigned region and conduct home visits as needed
