Help at Home is seeking a dedicated and compassionate Registered Nurse to join our team as a Case Manager. As a key member of our team, you will be responsible for supervising the personal care, companion, and skilled nursing services provided by our direct care staff. Your primary focus will be ensuring the quality of care for our clients in assigned areas.
Key Responsibilities:- Supervise and coordinate the delivery of skilled nursing services to clients in assigned areas.
- Conduct regular supervisory visits to clients, documenting visits and providing feedback to direct care staff.
- Develop and implement individualized care plans for clients, ensuring their needs are met and exceeded.
- Collaborate with branch staff, direct care staff, and family members to ensure seamless care delivery.
- Perform written annual competency evaluations of direct care staff, providing constructive feedback and recommendations for improvement.
- Assess and recommend solutions to address risks and concerns related to client care, staff performance, and organizational operations.
- At least 24 months of prior nursing experience, preferably in a multiple location environment.
- Knowledge of health administration regulations, health information management, and Medicaid, Waiver, and Home health service lines.
- Advanced knowledge of Microsoft Office Suite and ability to learn new applications.
- Flexibility and ability to prioritize work with minimal supervision in a fast-paced, high-productive environment.
