About the Role

We are seeking a dedicated and compassionate Registered Nurse to join our team as a Hospice Case Manager. In this role, you will provide direct care and support to patients and their families, working collaboratively with our interdisciplinary team to ensure a seamless and compassionate care experience.

Responsibilities
  • Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans.
  • Collaborate with the healthcare team to ensure timely and effective delivery of care, including medication administration and treatment as prescribed.
  • Counsel patients and families on meeting their nursing and related needs, providing emotional support and guidance throughout the care journey.
  • Document patient progress and maintain accurate clinical notes, ensuring compliance with regulatory requirements.
  • Participate in interdisciplinary team meetings to discuss patient care and develop strategies for optimal outcomes.
Qualifications
  • Graduate of an accredited nursing school with current licensure to practice in the state of operation.
  • 1 year of nursing experience, with 3 years preferred.
  • Experience in hospice or a similar setting is highly valued.
  • Knowledge of the hospice philosophy of care and commitment to clinical and documentation excellence.
