Job Summary:The Registered Nurse Case Manager will be responsible for planning, organizing, and directing hospice care with an emphasis on community health education and experience. This role requires a strong understanding of hospice care philosophy and the ability to work with patients and families to determine their needs and develop individualized care plans.
Key Responsibilities:- Conduct comprehensive assessments of patients and families to determine hospice needs and develop individualized care plans.
- Coordinate patient care for assigned caseload, including counseling and health education.
- Provide health care instructions to patients and families as needed.
- Assist patients with activities of daily living and facilitate self-sufficiency and comfort care.
- Maintain accurate and relevant clinical notes regarding patient condition and care provided.
- Communicate with physicians and hospice care team regarding patient needs and report changes in patient condition.
- Provide a safe environment for patients and arrange equipment and services as needed.
- Graduate of an accredited school of nursing with 1-2 years of recent acute care experience in an institutional setting and 1-2 years of recent experience in home care.
- Bachelor's degree with one year of hospice care experience preferred.
- Management experience not required, but will be responsible for supervising hospice aides.
- Nursing skills per competency checklist.
- Able to lift, position, and/or transfer patients and lift supplies and equipment.
- Willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs.
- Mileage reimbursement plan.
- Professionalism, compassion, empathy, and a desire to help others.
- Self-directing with the ability to work with little direct supervision.
