Nurse Case Manager

At Sacred Journey Hospice, we are seeking a compassionate and skilled Registered Nurse to join our team as a Case Manager. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and support to individuals and their families.
Responsibilities:- Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans.
- Collaborate with the healthcare team to ensure seamless care coordination and effective communication.
- Administer medications and treatments as prescribed by the physician, while also providing emotional support and guidance to patients and their loved ones.
- Maintain accurate and detailed clinical notes, while also participating in interdisciplinary team meetings to discuss patient progress and address any concerns.
- Ensure compliance with all relevant laws, regulations, and standards of care.
- Graduate of an accredited nursing program with current licensure to practice in the state of operation.
- At least one year of nursing experience, with three or more years preferred.
- Experience in hospice or a similar setting is highly desirable.
- Strong knowledge of the hospice philosophy of care and commitment to clinical and documentation excellence.
