Help at Home is seeking a dedicated and compassionate Registered Nurse to join our team as a Case Manager. As a key member of our team, you will be responsible for supervising the personal care, companion, and skilled nursing services provided by our direct care staff. Your primary focus will be ensuring the quality of care for our clients in assigned areas.
Key Responsibilities:- Supervise and coordinate the delivery of skilled nursing services to clients in assigned areas.
- Conduct regular supervisory visits to clients, documenting visits and providing feedback to direct care staff.
- Develop and implement training programs for caregivers to ensure they have the necessary skills to provide high-quality care.
- Collaborate with branch staff, direct care staff, and family members to ensure seamless care coordination.
- Perform written annual competency evaluations of direct care staff.
- Identify and recommend solutions to potential risks for clients, staff, and the organization.
- At least 24 months of prior nursing experience, preferably in a multiple location environment.
- Knowledge of health administration regulations and standards, health information management, and Medicaid, Waiver, and Home health service lines.
- Advanced knowledge of Microsoft Office Suite and ability to learn new applications.
- Flexibility and ability to prioritize work with minimal supervision in a fast-paced environment.
