Make a Difference: Patient-Centered Care Provider

Kinetix is seeking a compassionate and patient-focused Patient-Centered Care Provider to join our healthcare team. As a key member of our healthcare staff, you will play a vital role in providing exceptional patient care and advocating for the well-being of our patients.

About the Role:
We are searching for an experienced RN who is passionate about delivering high-quality patient care and has a strong commitment to patient-centered care. In this position, you will be responsible for providing direct and telephone nursing care, health teaching, patient advocacy, supervision, and coordination of care given by others. You will work closely with patients, families, caregivers, and other healthcare professionals to develop and implement individualized plans of care that meet the unique needs of each patient.

Responsibilities:
- Utilize evidence-based assessment techniques and complex interviewing skills to collect pertinent patient data
- Develop and continuously update individualized plans of care based on patient needs, condition, or requests
- Prioritize interventions and implement plans of care in partnership with patients, families, caregivers, and other healthcare professionals in a timely manner
- Conduct regular reevaluations and follow-ups to ensure optimal patient outcomes
- Demonstrate patient advocacy through collaboration with healthcare teams and effective communication with patients and families

Requirements:
- Bachelor's Degree in Nursing (BSN) or Associate's Degree in Nursing with acceptance or current enrollment in a BSN program and signed agreement
- Current licensure with the Board of Nursing for the State of Texas or Nurse Licensure Compact
- Basic Life Support (BLS) certification from the American Heart Association required
- 1 year of professional nursing experience preferred

Salary: $65,000 - $85,000 per annum, depending on experience and qualifications
