Primary Care Physician for Texas Center

We are seeking an experienced Primary Care Physician to join our team at CenterWell Senior Primary Care. Located in Texas, this is a permanent opportunity that offers a total compensation package exceeding $300K depending on experience. The ideal candidate will be bilingual in Spanish and have a strong background in Family Medicine, Internal Medicine, or Geriatric Medicine.
This role involves evaluating and treating patients in accordance with standards of care, following level of medical care and quality for patients, and monitoring care using available data and chart reviews. You will also assist in the coordination of patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care.
The successful candidate will work closely with the Regional Medical Director and Center Administrator to set a tone of cooperation in practice by displaying a professional and approachable demeanor. You will complete all medical record documentation in a timely manner and work with a quality-based coder to optimize coding specificity.
We are looking for a physician who is fully engaged in the concept of integrated team-based care and willing to learn/adapt to practice in a value-based care setting. Your excellent verbal and written communication skills, along with your ability to demonstrate high-level interpersonal relationships and communications with colleagues/patients, will be essential in this role.
The ideal candidate will have a minimum of two to five years of directly applicable experience, preferably managing Medicare Advantage panels of patients in a coordinated care environment. Knowledge of Medicare guidelines and coverage, as well as HEDIS quality indicators, is highly desirable.
