Nurse Care Coordinator

Job Summary:
Brown University Health, part of Lifespan, is seeking a skilled Registered Nurse to join our team as a Nurse Care Coordinator. This is a per diem opportunity with a competitive hourly rate of $45-$55, depending on experience and qualifications.
About the Role:
The Nurse Care Coordinator will work under the supervision of the Nurse Manager and within hospital and departmental policies and procedures. As a patient advocate, the RN will apply the nursing process to provide high-quality patient care. This includes recognizing and reporting deviations from patient or established norms, identifying emergency situations, and initiating appropriate responses and interventions.
Responsibilities:
The Nurse Care Coordinator will be responsible for:
- Providing basic comfort and support according to patient needs
- Fostering patient and family participation in achieving optimum patient functioning and performing activities of daily living
- Identifying and assessing emergency situations and initiating appropriate responses and interventions
- Defining nursing diagnosis and expected patient outcomes
- Implementing nursing interventions and evaluating patient outcomes
- Maintaining knowledge of contemporary professional nursing practice within the legal framework of the Rhode Island Nurse Practice Act
- Participating in quality improvement processes and risk management activities
- Integrating complete physical assessment with nursing assessment and records reviews to develop a comprehensive and individualized plan of care
- Maintaining a safe environment for patients, visitors, and staff
- Providing functional guidance and direction to LPNs, CNAs, and other licensed personnel as required
- Assuming shift charge responsibility on an assigned unit
Requirements:
To be successful in this role, the Nurse Care Coordinator must have:
- Graduate of a school of nursing with a current license to practice as a Registered Nurse in the State of Rhode Island
- Understanding of the provisions of the Nurse Practice Act of the state of Rhode Island
- Certification in cardiopulmonary resuscitation upon hire
- General proficiency as a staff nurse within three months of employment
- Specialty unit experience preferred
Work Environment:
The Nurse Care Coordinator will work in a fast-paced healthcare environment with prolonged standing and walking, exposure to all patient elements, and potential lifting and turning of patients and equipment.
