Job Summary

FSRI is seeking a dedicated Care Coordinator to join our team at Quanacut House. As a key member of our residential program, you will provide case coordination to clients, families, and collaterals, advocating for their needs and navigating human services and other departments.

Key Responsibilities:

  • Collaborate with multiple providers, including DCYF, schools, medical providers, and other service providers, to ensure the delivery of services to clients and families.
  • Monitor residential programs and maintain Trauma Systems Therapy (TST) in the milieu setting.
  • Develop and implement case plans, ensuring the needs of clients and families are met.
  • Provide support and guidance to clients and families, promoting independence and self-sufficiency.

Requirements:

  • Bachelor's degree in a social work-related field or significant residential/childcare experience.
  • Excellent communication and interpersonal skills.
  • Ability to work flexible hours, including evenings and weekends.
  • Knowledge of cultural, ethnic, racial, and socioeconomic issues.
  • Current driver's license and reliable transportation.

Physical Requirements:

  • Ability to travel to clients' residences, community locations, and office sites.
  • Ability to lift up to 20lbs.
  • Ability to communicate effectively and use sight and hearing.
  • Ability to push or pull 10lbs or more.
