Job Summary

We are seeking a highly skilled Clinical Liaison/Care Coordinator to join our team at CharterCARE Health Partners. This role is responsible for managing and coordinating inpatient rehabilitation services, ensuring compliance with regulatory and licensing standards.

Key Responsibilities
  • Develop and implement effective discharge planning strategies to ensure seamless transitions for patients.
  • Collaborate with healthcare teams to provide high-quality, cost-effective rehabilitation services.
  • Establish and monitor policies and procedures to ensure compliance with regulatory requirements.
  • Lead performance improvement initiatives to enhance patient outcomes and satisfaction.
  • Recruit, train, and supervise staff to ensure optimal team performance.
Requirements
  • Master's degree in Nursing, Rehabilitation, or Social Work (or related field).
  • Minimum three years' experience in rehabilitation care management.
  • Strong organizational and prioritization skills.
  • Excellent communication, collaboration, and negotiation skills.
  • Ability to make strategic decisions and assume responsibility for judgment.
