Overview:


Under the general guidance of the Director of Patient Safety and Accreditation, the Patient Safety Registered Nurse Specialist plays a pivotal role in enhancing the patient safety program, dedicated to the mission of eradicating preventable patient harm.

This role involves conducting daily assessments, initial triage, and referrals of safety incidents reported through the Vigilanz reporting system.

The specialist investigates patient safety occurrences through prompt fact-finding (including medical record reviews and interviews) and facilitates improvement initiatives as directed by the Director of Patient Safety and Accreditation.

Providing clinical expertise to the Patient Safety, Risk, and Quality teams, the incumbent creates objective and consistent summaries of findings when examining a safety incident or process.

Additionally, the Patient Safety Specialist RN assists the Director in overseeing ongoing analyses to ensure the sustainability of action items and mitigate the risk of patient harm.

This position requires building and maintaining strong relationships with colleagues, department leaders, clinical risk, and quality teams, as well as the patient safety organization, to support clinical excellence and patient outcomes.

Collaboration across ambulatory clinics and inpatient settings is essential to promote patient safety initiatives. The ideal candidate is a team player who can work independently with minimal supervision to coordinate, implement, and manage assigned tasks. Excellent communication skills are necessary for effective interaction with operational leadership and frontline staff. Adaptability is crucial to manage shifting priorities.

Responsibilities:
Operations and Investigation

  • Conducts initial assessments and referrals of safety incidents reported through the safety event reporting system, escalating urgent patient safety concerns to the System Director of Patient Safety and Accreditation and/or Clinical Risk Manager.
  • Facilitates safety event triage meetings and assists with collaborative referrals and classification of safety events.
  • Reviews clinical documentation in the electronic medical record and engages in various fact-finding methods to ensure appropriate safety event referrals, follow-ups, and closure.
  • Conducts safety event reviews, including root cause analyses as directed by the Patient Safety, Quality, and Risk teams.
  • Utilizes process improvement techniques, systems thinking, and knowledge of human factors to develop action plans aimed at preventing the recurrence of patient safety incidents.
  • Promotes the Patient Safety Program through presentations, colleague training, and participation in relevant committees.
  • Performs Failure Modes and Effects Analysis as assigned by the patient safety, quality, and risk teams.
Safety Culture

  • Identifies trends in reported safety incidents and compiles meaningful and actionable data for dissemination to department and senior leaders.
  • Utilizes resources such as the Joint Commission and the ECRI Patient Safety Organization (PSO) to inform proactive rounding and collaborate with department leaders in implementing best practices for patient safety, as well as reviewing, prioritizing, and creating action plans based on department-level safety incident data.
Documentation

  • Compiles objective summaries and timelines of events to inform stakeholders in determining if a deviation from intended processes or system vulnerabilities exist.
  • Uploads updates on safety event reviews, root cause analyses, and associated action plans to the safety event reporting system.
Compliance/Safety

  • Ensures all certifications and/or licenses are current and valid prior to expiration dates, if required for the position.
  • Completes all mandatory company modules and required job-specific training within the specified timeframe.
  • Responsible for timely reporting of any safety-related incidents through the Vigilanz system; attends all safety-related training programs; performs work safely; monitors the work environment for potential safety issues and ensures compliance among colleagues.
  • Maintains up-to-date knowledge of cybersecurity trends, developments, best practices, and regulatory changes.

Qualifications:
Education

Bachelor's of Science in Nursing - Required

Master's Degree - Preferred

Certification & Licensures

Active, unrestricted RN License from any state (AZ preferred) – Required

RCA2 IHI Training within 1 year of hire - Required

Lean Six Sigma Greenbelt - Preferred

Certification in Patient Safety - Preferred

Lean Six Sigma or other process improvement methodology - Preferred

Experience

Minimum of 3-5 years of acute care experience as an RN - Required

Experience in process improvement and/or project management - Preferred

In a rapidly evolving healthcare environment, technology is integrated into nearly all aspects of patient care. Proficiency in using computers and basic software programs is essential.
