Job Title: Hospice Social Worker

We are seeking a qualified Hospice Social Worker to join our team. In this role, you will work closely with patients, families, and healthcare providers to deliver high-quality psychosocial support.

Responsibilities
  1. Conduct thorough psychosocial assessments to identify patient needs and develop personalized plans of care.
  2. Collaborate with healthcare teams to ensure seamless delivery of patient services.
  3. Develop and implement counseling strategies to address stress and coping difficulties.
  4. Maintain accurate records of patient interactions, services provided, and outcomes achieved.
  5. Facilitate communication between patients, families, and community agencies.
Requirements

To succeed in this role, you must have:

  • A Master's Degree in Social Work (MSW) from an accredited institution.
  • Current State Licensure in Social Work (LMSW or LCSW).
  • A minimum of one year's experience in health care or hospice settings.
  • A valid driver's license and proof of insurance.
Compensation and Benefits

We offer a competitive salary range of $70,000 - $90,000 per annum, depending on experience, along with a comprehensive benefits package, including medical, dental, vision, life/AD&D, disability, 401(k) retirement plan with company match, generous time off accruals, mileage reimbursement, paid holidays, tuition reimbursement, employee referral program, merit increases, and employee discount programs.
