About the Role

We are offering an immediate opening for a Social Worker who will be responsible for providing exceptional patient support and psychosocial assessments to patients and families in our hospice program. The ideal candidate should have a Master's Degree in Social Work, current State Licensure in Social Work, and at least one year of experience in healthcare or hospice settings.

Key Responsibilities

• Conduct thorough psychosocial assessments of patients and families/caregivers to identify their needs and develop personalized plans of care.
• Counsel patients and their loved ones to address stress, coping difficulties, and other emotional challenges associated with terminal illnesses.
• Collaborate with community agencies and healthcare professionals to provide comprehensive support and resources to patients and families.
• Prepare accurate and timely documentation of patient services to ensure seamless care coordination.
• Provide assistance with applications for Supplemental Security Income, Medicaid, and Medicaid Waiver programs to help patients access necessary benefits.
